چکیده

Purpose: Two-dimensional (2D) high-dose GRID radiotherapy has shown effective tumor control in the clinical setting, and radiobiological data suggest potent bystander/abscopal effects with this technique. We present a new technical concept using modern radiation therapy instrumentation to advance traditional GRID treatment to modern three-dimensional (3D) high-dose LATTICE radiotherapy (LRT). Materials and Methods: An array of focused high-dose volumes, in essence a lattice of doses in 3D, can be generated through modern techniques resulting in highly heterogeneous dose distributions within the tumor volume, leaving adjacent and peripheral normal tissue minimally exposed. Two technical approaches were investigated for this novel concept: noncoplanar focused beams and MLC-based or aperture-modulated arc. Results: High magnitude dose oscillation is essential in traditional GRID therapy. The 3D "peak-to-valley" dose fall-off (100% to 20-30%) characteristic was attainable in test LRT cases, with a greater ability to place dose peaks within the radiation target volume and minimal dose to surrounding normal tissue. Conclusion: Modern radiotherapy methods are readily available to deliver 3D high-dose LATTICE radiotherapy with superior dosimetry compared to the 2D GRID technique. This is an easily accessible therapeutic modality that could potentially result in comparable or superior treatment outcomes than traditional 2D GRID therapy when implemented in the clinical setting. The Use of Lattice Radiation Therapy (LRT) in the Treatment of Bulky Tumors: A Case Report of a Large Metastatic Mixed Mullerian Ovarian Tumor.

The objective of this teaching case is to report the excellent results of using lattice radiation therapy (LTR) for the treatment of a large metastasis from ovarian carcinosarcoma. This new technical concept extrapolates the traditional spatially fractionated radiation therapy (GRID) technique to advanced three-dimensional (3D) high-dose radiation therapy using modern instrumentation in radiati...

Lattice Radiotherapy with RapidArc for Treatment of Gynecological Tumors: Dosimetric and Early Clinical Evaluations

Background: We report the case of a patient with an advanced carcinoma of the cervix treated using the new concept of LATTICE Radiotherapy (LRT). We describe the dose distribution, planning and delivery of treatment as well as the clinical evaluation of the patient during and at the completion of treatment. The basic principle of LATTICE is to create, within the tumor volume, multiple localized...

Comparison of late rectal toxicity from conventional versus three-dimensional conformal radiotherapy for prostate cancer: analysis of clinical and dosimetric factors.

OBJECTIVES To compare late rectal toxicity (LRT) after definitive radiotherapy (DR) and salvage radiotherapy (SR) in prostate cancer using conventional (CONV) or three-dimensional conformal (3-D) techniques. METHODS The outcomes and clinical factors of 212 patients with Stage T1a-T4 prostate cancer were evaluated (separated into DR and SR groups). The median prescribed dose was 66, 74, 66, an...

Assessment and Comparison of Homogeneity and Conformity Indexes in Step-and-Shoot, Compensator-Based Intensity Modulated Radiation Therapy (IMRT) and Three-Dimensional Conformal Radiation Therapy (3D CRT) in Prostate Cancer

Introduction: Intensity modulated radiation therapy (IMRT) and three-dimensional conformal radiation therapy (3D CRT) are two treatment modalities in prostate cancer, which provide acceptable dose distribution in tumor region with sparing the surrounding normal tissues. IMRT is based on inverse planning optimization; in which, intensity of beams is modified by using multileaf c...

Effect of Bolus Frequency and Its Thickness in Postmastectomy Three-dimensional Conformal Radiotherapy on Skin Dose for Superposition Algorithm

Introduction: The postmastectomy radiotherapy uses bolus to improve the coverage close to the skin; however, it needs to be removed in case of severe skin toxicity. This study investigated the effect of bolus parameters (i.e., frequency and thickness) for the superposition algorithm on skin dose in postmastectomy three-dimensional conformal radiotherapy (3D-CRT). <str...
